PDF format. Once you have have completed and signed the form (in two places!), please return it to the Honorary Membership Secretary (address on the form).
If you do not know any members who can Propose and Second you, please indicate this when returning the form and we will contact you.
We stress that any information you disclose on the form will only be used in connection with your membership and will not be passed to any third party individual or organisation.
